Complete the Transcript Request Form.

Once you have filled out the request form, please mail it to Thalia Bozarth at the address below, and include cash or a check in the amount owed (depending on the number of transcripts requested), as well as a self-addressed stamped envelope(s) of where you want the transcript(s) to be sent or with a cost of current stamp value. If you would like an unofficial unsigned copy you can email the request to Tbozarth@asjusd.org. You may also come in to the office in person to request a copy.
