California Healthy Kids Survey (CHKS)

Survey Information

The California Healthy Kids Survey (CHKS) is the largest statewide survey on student well-being, providing insights into the link between health and academic success. Used by schools and policymakers, CHKS helps guide programs for health, prevention, and school improvement. Results are compiled into district and county reports to support data-driven decision-making. To view a copy of your district’s report, go to https://calschls.org/reports-data/search-lea-reports/ and type in the district name.

Parent Consent

In accordance with California Education Code 51513 and 51938(c), CHKS requires different types of parent/guardian consent based on grade level:

  • Active (Opt-In) Consent is required for students in grade K-6, meaning parents must provide written permission for their child to participate.

  • Passive (Opt-Out) Consent is used for students in grades 7-12, meaning parents will receive notification from their child’s school with the option to withdraw their child from participation if desired.

Survey Administration

For the 2024-25 school year, CHKS will be administered to students in grades 3-12. The survey window is scheduled for March 20 - April 15, 2025. Parents will receive advance notification with details and consent requirements.
